Asthma

Asthma Management Program

If you have asthma, our asthma management program will aim to:

  • Help you become active in your own care
  • Improve your health and quality of life
  • Help you understand your asthma symptoms and triggers

Behavioral Health

Behavioral Health Management Program

If you have been told you have Bipolar Disorder or Schizophrenia, our Behavioral Health Management program will aim to:

  • Help you become active in your own care
  • Improve your health and quality of life
  • Help you understand your condition

Chronic Obstructive Pulmonary Disease (COPD)

COPD Management Program

If you have COPD (also known as emphysema), our COPD management program will aim to:

  • Help you become active in your own care
  • Improve your health and quality of life
  • Help you understand your COPD

Diabetes

Diabetes Management Program

If you have diabetes, our diabetes management program will aim to:

  • Help you become active in your own care
  • Improve your health and quality of life
  • Help you keep your blood sugars at a normal level
  • Prevent or detect diabetes-related problems

Heart Disease and Heart Failure

Chronic Kidney Disease

If you been told you have kidney disease stage 3 or above/or on dialysis our Chronic Kidney Disease Management Program will aim to:

  • Help you understand your kidney condition

Cancer

Cancer Management Program

If you are being treated for breast, lung, colorectal, or a blood cancer our Cancer Management Program will aim to:

  • Help you become active in your own care
  • Help you understand and manage any symptoms and side effects of cancer treatment

What do these health programs include? A nurse will talk with you about:

  • How the diseases could affect you
  • How the treatments could affect you
  • Your overall health
  • Your feelings about the diseases
  • Your feelings about the treatments
  • Any medications you are taking
  • The importance of seeing your doctor
  • The importance of nutrition, blood pressure control, diabetes control, and weight management
  • Any other support services you may need

Follow-Up Calls

After the first call, we will sometimes check in to see how you are doing and to answer your questions. We will give you information and support to help you live a healthy lifestyle. In addition to your end stage renal disease, we may give you information about:

  • Healthy weight
  • Exercising
  • Planning meals
  • Quitting smoking
